Student Success & Support Program (SSSP) Forms 

Please select ONE appropriate form below to fill out:

Release of Enrollment Hold –

Exemption from Assessment

This form should be used by students who are planning on continuing their education with us at College of Alameda and would like to be exempt from our assessment because they have completed it elsewhere (i.e. other college transcripts, other college assessment results, etc.).

 

Please remember, although you may be exempt from our assessment, you must still complete our mandatory online orientation prior to submitting this form.

 

 

Release of Enrollment Hold –

Exemption from all SSSP Services

This form should be used by students who would like to exempt from ALL of the SSSP services because they meet any of the following criteria below:

  • A student who has earned an AA/AS degree or higher
  • A student who is attending a non-PCCD college and is taking a PCCD course to satisfy course requirement at primary institution (i.e. spring/winter intersession course).
  • A student who is not planning on transferring or obtaining a certificate of degree from a PCCD college or pursing career development at a PCCD college
  • A student who has to take a course that is mandated for employment.

Reminder: If you exempt from all the SSSP services, you will lose your priority registration.

Prerequisite Clearance Request Form

Before registering for your classes, please check to see if your class has a prerequisite requirement (http://alameda.peralta.edu/college-catalog/files/2015/08/College-of-Alameda-2015-2017.pdf). All prerequisites must be cleared before enrolling into classes.

 

Note: If you have an Enrollment Hold on your account, please fill out the appropriate “Release of Enrollment Hold” Exemption Form which can be found above.
